Central Coast Home Health and Hospice is a fast-growing and locally owned organization with an excellent reputation throughout the San Luis Obispo and Santa Barbara counties. We are constantly growing and now seeking an Intake Coordinator to join our Hospice Intake team in the San Luis Obispo office.

Work schedule: fulltime hours, Monday through Friday 8-5, with a weekend rotation


Duties:

  • Assists with daily patient referral and intake operations including providing oversight of the establishment
  • Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements
  • Assists with the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy
  • Identifies appropriate internal and external referral sources, MDs and disciplines needed for scheduling the referral
  • Establishes and maintains positive working relationships with current and potential referral sources
  • Identifies and distributes faxes to appropriate personnel

Qualifications:

  • Must have previous work experience in a busy office environment; previous experience in a healthcare setting preferred
  • Excellent written & verbal communication skills
  • Able to type at least 40 wpm
  • Advanced knowledge of Microsoft programs (Outlook and Word)
  • Must be able to keep internal business information confidential
  • Have a customer-service based approach with all interactions
  • Work well independently and in a team environment
  • High attention to detail and good follow up
